Quality audits and monitoring

Understand your service. Act on the findings.

Quality monitoring gives owners and managers a clearer view of what is working, where practice is inconsistent and what needs to improve.

We provide independent reviews with clear findings and practical recommendations. Reviews can be commissioned individually or included in an ongoing consultancy arrangement.

What we can review

Depending on your service and agreed scope, audits may cover:

  • Care and support plans.
  • Individual risk assessments.
  • Safeguarding arrangements and records.
  • Medication records and associated management processes.
  • Recruitment and workforce documentation.
  • Induction, supervision and training.
  • Incidents, complaints and learning.
  • Feedback from people using services, families and staff.
  • Policies and their application in practice.
  • Record quality and care management systems.
  • Governance and management oversight.
  • Progress against previous actions.

Focused or wider reviews

You may need a themed audit of one concern or a broader assessment of your service.

We agree the areas to be examined, the evidence required and how findings will be presented.

Reviews may involve document examination, discussions, observation and visits, depending on the scope.

What you receive

Our reports explain the evidence reviewed, key findings and recommended actions.

We help you prioritise improvements, assign responsibilities and establish how progress will be checked. Follow-up support can help managers implement changes and review whether they are working.

Mock inspections

Mock inspections help teams examine their readiness against the relevant regulatory framework.

They provide an opportunity to identify gaps, discuss practice and organise evidence. They do not predict or guarantee a regulator’s judgement.

Support after a Requires improvement or Inadequate rating

A Requires improvement or Inadequate rating, or a service being placed in special measures, needs a clear and prompt response.

We help you understand the findings, prioritise the actions that matter most and build an improvement plan with clear responsibilities and timescales. We then support managers to put changes in place, organise evidence of progress and prepare for re-inspection.

  • Review of inspection findings and any enforcement action.
  • Improvement and action plans.
  • Evidence of progress for the regulator and commissioners.
  • Follow-up audits to check changes are working.
  • Management mentoring through the improvement period.
  • Preparation for re-inspection.

The regulator decides ratings and enforcement action. Where legal advice or representation is needed, we can work alongside your legal adviser.
